Management of a vascular complication: A case report

Vascular compromise is a rare but serious complication of hyaluronic acid (HA) filler injections, especially in the nasolabial fold due to its complex vascular anatomy. We present a 45-year-old female who developed moderate pain and livedo reticularis after HA filler injection in our clinic. Capillary compression was diagnosed the next day. High-dose pulsed hyaluronidase was administered in two sessions: a first dose on day 2, followed by a second dose the next day due to incomplete clinical response. Acute symptoms resolved within 14 days without necrosis, although vascular dilation persisted for 3 months. This case emphasizes the importance of early recognition, prompt management, and knowledge of facial vascular anatomy to prevent and manage complications in aesthetic practice.
